How do I apply?
If you are aged between 16 and 40 (inclusive), fit our medical criteria and are resident in the UK, all you need to do is fill out an official special day application form.
Download the Special day application form, or alternatively call 01707 259777 to request a copy to be sent to you in the post.
You will need to ask a medical professional (e.g. a doctor, nurse or social worker) who knows you and is involved in your treatment, to fill out and sign section three on the form. You will also need to give details of a 'responsible companion' - someone aged 18 or over who will accompany you on your special day. They must also sign the form.
Please return the application form by post or fax (as we must have your signature and that of your medical professional). A member of the Foundation's special days team will be in contact very soon.
